Effective March 26, eligibility is based exclusively on manufacturer vaccine age recommendations only. Therefore, anyone 16 or older is eligible for vaccines. This is subject to change based on vaccine availability and IDPH direction at any given time.

What should I bring with me for my vaccine appointment and when should I arrive?
When possible print and complete the forms below; bring them with you on the day of your appointment. Also, COVID-19 vaccines are at no cost, but participants should bring current health insurance card if they are covered under an insurance plan.

 
Participants should arrive no earlier than 15 minutes before their scheduled vaccination time. 
 
How do I know if I am eligible? 
Eligibility is based on criteria established by the Illinois Department of Public Health. See below for eligibility. Must be an Illinois resident.
