Freedom of Information Act
The Freedom of Information Act 2014 provides that every person has the following legal rights:
- A legal right for each person to access information held by public bodies
- A legal right to have official information relating to him/herself amended where it is incomplete, incorrect or misleading
- A legal right to be given reasons for decisions affecting them.
There are exemptions provided for in the Act. This means that there are specific circumstances when the requested information will not be released. e.g. to protect confidentiality.
GENERAL DESCRIPTION
Hamilton Memorial Hospital District is a Critical Access, 25-bed facility located in McLeansboro, IL. Hamilton Memorial Hospital has been serving Hamilton County and the surrounding areas since 1960. Staffed by a team of over 180 professionals, HMHD provides comprehensive patient-focused care with state-of-the-art technology, close to home.
HMHD offers inpatient acute and transitional care, along with a fully staffed 24/7 Emergency Department for emergent needs. There are two family clinics, one in McLeansboro, IL and one in Carmi, IL, along with a Senior Enrichment center that provides education, support and counseling for seniors. Our Family Clinics offer not only primary care services, but also host specialty services to receive specialized care at our modern, technologically advanced facilities. Hamilton Memorial Hospital District is dedicated to providing quality healthcare options with the convenience of being close to home for our community.
TYPES OF PUBLIC RECORDS MAINTAINED BY THE HOSPITAL INCLUDE THE FOLLOWING:
GENERAL
Board meeting schedules, board minutes and resolutions, policies and procedures, legal notices, employee names, titles, dates of employment, official bonds, records of ownership of real or personal property, and contracts.
FINANCIAL
Annual budgets, tax levies, audit reports, bills or invoices issued and received by the hospital, receipts for revenue.
Note: Exemptions under the Illinois Freedom of Information Act may allow the non-disclosure of some parts of public records maintained by the Hospital.

You may download this form by clicking here, or you may request it at any facility. You are NOT required to use this form to submit a request. You may submit a FOIA request in any written format that you choose.
The hospital will provide electronic records when possible. No fees shall be charged for the first 50 pages of black and white, letter or legal-sized copies requested.
After the first 50 pages, the fee for black and white, letter or legal-sized copies shall be 15ยข per page.
Actual cost will be charged for other documents not of standard size and for the recording medium (e.g. compact disk, tape, DVD). The hospital may waive or reduce fees if the person requesting the records states the specific purpose for the request and indicates that a waiver or reduction of fees is in the public interest.
