Hamilton Memorial Hospital District Recognized for Most Improved Annual Wellness Visit Performance
Hamilton Memorial Hospital District is proud to announce that its clinic team has been recognized with the Most Improved Annual Wellness Visits (AWV) Award for its performance in 2025.
An Annual Wellness Visit is a preventive visit designed to help patients and their healthcare teams focus on maintaining good health and identifying opportunities for preventive care. Unlike a traditional physical exam, an AWV focuses on prevention and creating a personalized plan based on each patient’s individual health needs.
During an Annual Wellness Visit, the healthcare team may review preventive screenings and recommended services, medical history and health risk factors, medications, immunizations, health measurements, fall risks, cognitive or depression screenings when appropriate, advance care planning, and individual health goals.
This recognition represents more than an increase in the number of Annual Wellness Visits completed. It reflects the coordinated efforts of providers, nurses, scheduling staff, and others who work together to identify patients who are due for an AWV, reach out to them, schedule appointments, and ensure they receive recommended preventive care.
Increasing Annual Wellness Visits also supports Hamilton Memorial Hospital District’s efforts as part of its Accountable Care Organization (ACO). By focusing on preventive care and identifying gaps in recommended services, the team can help support improved patient outcomes and healthier lives.
Hamilton Memorial Hospital District thanks its providers, nurses, scheduling staff, and everyone who contributed to this achievement. The team looks forward to building on this progress and continuing to make preventive care a priority for the patients and communities we serve.
