Requirements:

• Graduate of an accredited Physical Therapy School.
• Licensed in the State of Illinois.
• Current CPR card Knowledge of general P.T. practice act. Knowledge of Rehab/PT policy and procedures.
• Capable of communicating effectively verbally or in writing.
• Must have good leadership skills.

Description:

• Evaluates and records patient treatment plan in coordination with their physician.
• Establishes patient treatment programs to include modalities of heat, cold, massage, mobilization, hydrotherapy, light therapy, electricity and patient education as per orders from the physician.
• Evaluates gait patterns and instructs patients in proper technique and use of devices.
• Coordinates patient care or treatment with other members of the team according to abilities.
• Adapts treatments according to the level of improvement of the patient.
• Provides home health services through a contract with Addus or Egyptian Health Department.
• Performs other duties as required or assigned.
