Hamilton Memorial Hospital District (HMHD) provides patients with access to specific health information via an online medical record. If you are admitted as an inpatient at HMHD, or have outpatient lab tests done, once you are discharged you will be able to view the following information online:
Active and inactive medications
Current and past health concerns
Certain test results
A limited number of same-day test and procedure results will also be available online. Please note, you will be able to access information from hospital inpatient stays and limited same-day tests and procedure results.
Register Your Account
These instructions will guide you through the registration process. It is important to follow these instructions in the order they are given.
Step 1: Confirmation Email
Check your email inbox or spam folder for the confirmation message to begin your registration process. The email will be sent from firstname.lastname@example.org and the subject line will state “Hamilton Memorial Hospital – Patient Portal New User”. Open the email and click the link provided. You will be transferred to a webpage where you will be asked to register as a new user.
Step 2: Registration
You will be asked to enter your name, date of birth, unique user ID, password and choose an avatar/picture.
Very important: Make sure you check the box at the right to verify that your information is correct. Then click the “register button” at the bottom of the screen.

Step 3: Security Questions
Choose 3 security questions, fill in answers and then click the “submit” button at the bottom of the screen.

On the next page, click “Main Menu” to begin looking at your medical record.
Step 4: Patient Record
From the patient list, click on the name of the patient you are authorized to view. The patient’s visits will be listed chronologically. Next to each visit there is a link that shows the account number and the title “Summary of Care”. Click on the link in the Summary of Care column to view the document.
Step 5: Log Out
To log out from your session, click the homepage symbol. Once there, find your avatar/picture in the upper left of the page and click on it. A drop down will appear and give you the option to log out. To log back in next time, you will go to
